Complete this form when you have dealings with the Ontario Ministry of Revenue and you need to authorize the ministry to deal with another individual as your representative for EHT or to cancel one or more existing authorizations. - Schedule 1 - Multiple Accounts
An eligible employer with multiple accounts must designate one account to complete and submit this schedule. - Schedule 2 - Associated Employers Exemption Allocation
Schedule 2 is used to calculate and allocate the exemption available to the associated group for the year.
